How to access support

To request a referral you will need to send us a completed ‘Preliminary Application Form". The form is below ready to download. If you have any difficulties completing the form, we are happy to help.

Once we have received the Preliminary Application, a manager will contact you to discuss the kind of support you would like.

If it is decided to take the application forward and we feel that we can provide the support you need, we will carry out an assessment. We will meet with the Adult, Child/Young Person, relevant family members and professionals to discuss the provision of a person centred service, tailored to the individual.

Funding

We accept referrals from direct payment, individual budgets and local authorities.
